How to read CMS warning flags
- Special Focus Facility (SFF)
- A nursing home CMS has identified with a persistent record of serious quality problems. It is inspected about twice as often. CMS nursing homes page (SFF list in Downloads).
- SFF candidate
- A home whose inspection history puts it on the list CMS selects Special Focus Facilities from.
- Abuse citation
- CMS shows this when a home was cited for abuse, neglect or exploitation in recent inspections. Medicare explanation.
- Fines and payment denials
- Federal penalties from the last three years of inspections.
